Our Team
Adult Psychiatirst/Medical Director
Dr. Soto-Acosta is a Board-Certified Adult Psychiatrist, who provides psychiatric and substance use care. He is fluent in English and Spanish. He is currently not accepting new patients.
FNP, PMHNP-BC, CDE
Lavern is an experienced Dual Board-Certified NP (Psychiatry and Family Medicine) and a Certified Diabetes Educator who is fluent in English and Spanish. She provides diabetes care, psychiatric services and family practice services. She accepts patients 8 and up.
PMHNP-BC
Kristin is a Board-Certified Psychiatric NP who takes a patient-centered approach to treatment. She provides evaluations and medication management for mental health and substance abuse. She accepts new patients ages 16 and up.
Chuck is a Licensed Clinical Social Worker who provides counseling services to patients 18 and up, utilizing a variety of therapeutic modalities. He is also a Diplomate of the American Board of Forensic Counselors.
